    • Fertility benefit – Our fertility benefit is available for all benefit-eligible team members, regardless of whether you elect health plan coverage. The benefit is designed to meet the individual needs of you and your family and offers options to meet you where you are at in your fertility journey. Covered services include fertility planning and diagnostics, reproductive technology to include invitro fertilization (IVF), intrauterine insemination (IUI), egg/sperm retrieval and/or freezing and gamete donation and transport. Coverage is also available for adoption and surrogate services. The benefit offers a lifetime maximum for you or your spouse up to $20,000.

    Benefits Eligibility

    Medical, dental, vision, health savings account and flexible spending accounts are available for full-time and part-time employees scheduled to work at least 20 hours per week.
    Life insurance, disability insurance, critical illness, hospital indemnity, legal, identity theft, group accident and whole life insurance is available to full-time and part-time employees scheduled to work 16 hours per week.
    Employees scheduled to work 30 or more hours per week are considered full-time.
    Children’s offers insurance benefits to spouses, regardless of availability of other health insurance.
